Space-filling pessaries occupy the vaginal vault and include Gellhorn, cube, and donut type devices (Figure 2). In our practice, typically these pessaries are used more commonly for patients with stages 3 and 4 prolapse, or when support-type pessaries fail.

If a cystocele or rectocele accompanies the third-degree uterine prolapse, a Gehrung pessary may be the most helpful.

Can a pessary go in too far?

If the pessary is uncomfortable, you may take it out. You can reinsert it right away or leave it out for hours. Remember to push the pessary back as far as you can get it. You cannot push it in too far.

Can you push a rectocele back in?

Splinting, this is inserting a tampon or two fingers inside the vagina and pushing back. Surgical repair may be indicated if the rectocele is severe. A rectocele repair may be performed through the anus, vagina, perineum (between the anus and vagina), the abdomen, or may be a combined repair.

How do you know what size pessary you need?

Begin by inserting your middle finger behind the cervix in the posterior fornix and placing your index finger against the pubic notch. The distance between your 2 fingers is used as a starting point in pessary sizing. Withdraw your fingers and choose the fitting ring whose diameter best approximates this distance.
